2. Email this Emergency and Release form to all parents and adult sponsors:

(Youth and sponsors who have already completed this form for a presbytery-sponsored event in the 2025-2026 school year are not required to complete it again.)
3. Print this Gilmont Waiver of Liability. Obtain parents' signatures and bring these forms with you to GAP.
